Transition Training

This training will help families and professionals develop and implement a health care transition plan for youth and young adults with developmental disabilities.

Listed below are five training chapters that you will find very helpful in learning what you need to do for a successful transition. Each training chapter may contain several presentations, each approximately 15 minutes in length. After watching a presentation, you must close the presentation to return to the chapter homepage. Please view all presentations in the chapter before moving on to another chapter.

In this chapter you will learn about preparing for transition, negotiating the adult health care system, and providing ongoing assistance to young adults who have a significant intellectual impairment.
