A Young Person’s Guide to Health Care Transition
This 24 page guide is written for teens/young adults and families and has a separate section for youth in three different age ranges (12-14; 15-17 and 18+). Each section includes information to help youth be more in charge of their own health care, tips for parents, and a checklist of transition skills and activities. The guide also includes a list of transition related resources. Available in English and Spanish. Can be downloaded as a PDF color document or a printer-friendly grayscale document.

Health Care Transition Workbooks
Three age-linked HCT workbooks are available, (12-14; 15-17 and 18+) and are designed to help parents and youth with special needs look at future goals, assess teen’s level of independence on key health related activities, identify what parents are doing to support the teen’s autonomy in self-care, and develop a plan to transition from pediatric to adult-oriented health care. All workbooks are available in English and Spanish as a printer-friendly PDF.

This 18 minute video teaches teens how communicate effectively with health care providers so that they answer teens' questions and give the information and supports teens need to be more in charge of their own health. Teens will also learn how prepare and give health care providers the information they need to give teens the best possible medical care. (2008)

This is Health Care Transition
Growing up and becoming an adult is difficult for all youth, but can be especially challenging for young people with special health care needs. This video was developed to help youth and young adults with chronic health conditions and their families be better prepared for adulthood, especially the move from pediatric to adult-oriented health care. This video was developed through a contract from Children’s Medical Services to the University of Florida. Run time: 29 minutes. (2007)

In many ways, Jeff Walden is a typical college student, but in some ways he is not. He was born with a severe physical disability that has posed many challenges on his road to independence.
