This 24 page guide is written for teens and young adults and their families. The guide has a section for youth in three different age ranges (12-14; 15-17 and 18+). Each section includes age-related information that will help youth be more in charge of their own health care, tips for parents, and a checklist of transition related skills and activities. The guide also includes an extensive list of health care transition related resources. This document is available in English and Spanish languages and can be downloaded either as a PDF color document or as a printer-friendly grayscale document.

Health Care Transition Workbooks

Three age-linked versions of HCT workbooks are available, (ages 12-14; ages 15-17 and 18+ years) These workbooks are designed to help parents and their child with special needs look at future goals, assess the child’s level of independence on key health related activities, identify what parents are doing to support their child’s autonomy in self-care, and develop a plan to help assure the eventual transition from pediatric to adult-oriented health care goes smoothly. Each of the three workbooks is available in English and Spanish as a printer-friendly PDF.

Growing up and becoming an adult is difficult for all youth, but can be especially challenging for young people with special health care needs. This video was developed to help youth and young adults with chronic health conditions and their families be better prepared for adulthood, especially the move from pediatric to adult-oriented health care. This video was developed through a contract from Children’s Medical Services to the University of Florida. Run time: 29 minutes. (2007)
