The mission of the Health Care Transition Initiative at the University of Florida is to increase awareness of, gain knowledge about, and promote cooperative efforts to improve the process transitioning from child-centered (pediatric) to adult oriented health care. Our vision is to improve the transition process for all adolescents and young adults, although our current efforts focus on those with disabilities and special health care needs.
Education and Healthcare Transition Certificate
The University of Florida Colleges of Education and Medicine are offering a new graduate level, online certificate in Education and Healthcare Transition (EdHCT) designed for graduate students and professionals in medicine, nursing, education, social work, law, public health and public policy, physicians' assistants and other health-related areas. The program allows participants to learn new skills for integrating education and health care transition and building expertise in this emerging discipline. The certificate comprises 4 online courses that may be taken individually, but the certificate will be provided only if all are completed.

Health Care Transition Training Program for Professionals
This online training curriculum provides information about how professionals can help prepare youth with chronic health conditions and disabilities for their eventual graduation from pediatrics and transfer to adult health care.
