Health Care Transition Planning Guides

Three age-linked HCT planning guides are available, (12-14; 15-17 and 18+) and are designed to help parents and youth with special needs look at future goals, assess teen's level of independence on key health related activities, identify what parents are doing to support the teen’s autonomy in self-care, and develop a plan to transition from pediatric to adult-oriented health care.

The English version of all three planning guides can be completed online. The online version of the guides make it easy to fill in information and print out the plan for how a youth or young adult can be more in charge of their healthcare.
